The team have recently successfully completed a large Cataract surgery contract for West Wiltshire PCT. All patients were dealt with using a novel "one-stop" patient pathway that proved highly efficient, and popular with patients.
The patient pathway was as follows:
The patient arrived at the Hospital, and was assessed by a member of the Nursing staff. All the data, including biometric measuremnts for calculation of lens implant power were entered onto a dedicated electronic patient record. The patient then saw a member of the Consultant team, and clinically examined and fully informed consent obtained.
The patient was then escorted to the day-case area, and prepared for surgery. Following surgery, the patient was settled back onto the day unit, and after refreshment and nursing assessment, allowed to go home. A discharge summary and GP letter was generated at the time of surgery and sent to relevant parties.
The patient was seen once post-operatively, and if all was well, discharged from follow-up back to the care of their Optometrist and GP. All patients were asked to visit their Optometrist, and data from that visit was entered onto the electronic record.
